• Chicken, pork, and beef were stored in direct contact with ice in a prep-top cooler. (3 points)
• Food was stored uncovered beneath an unpackaged toilet seat. (3 points)
• Heavy buildup was observed on shelves, cooler surfaces, and equipment throughout the kitchen. (1 point, repeat)
• Buildup was observed on walls, ceilings, and floors throughout the facility. (1 point)
• The employee restroom door was propped open. It was closed during the inspection. (1 point)
• Low floor grout, torn ceiling tiles, and broken floor tiles in the walk-in freezer were observed. (1 point, repeat)
• Lighting was inadequate in kitchen and storage areas. (1 point)
• A fly light was installed over a food preparation area. (3 points)
• Multiple flies were observed throughout the kitchen, along with rodent droppings in the dry storage area. (3 points)
